Does anyone need a vascularized graft?
Many hand surgeons argue that vascularized bone grafting is indicated in proximal pole avascular necrosis, prior failed surgery, or non-standing nonunion.
There is no evidence to show significant differences in union rate or time to union between vascularized and nonvascularized grafting.
Current diagnostic tools do not accurately reflect proximal pole vascular status.
